Schedule your first member visit with our Medical Specialist. Here's what you can expect.

A1c Screening

We’ll begin with a quick finger prick to collect a small drop of blood, allowing us to test your A1c level in just a few minutes.

This A1c result will indicate whether you’re non-diabetic, prediabetic, or diabetic. Depending on the level, you may face increased risks of complications such as heart disease, stroke, vision loss, kidney failure, limb amputations, neuropathy, and premature death.

Medical Exam

We'll assess your weight, BMI, blood pressure, and heart rate. If you have any prior lab results or biometric data, we’ll review those as well. Otherwise, we'll draw your labs and start you on a biometric monitoring program.

A brief physical examination and systems review will help us evaluate any other potential health issues you may have.

We’ll discuss your medical history, known allergies, and any medications you’re currently taking. This comprehensive approach will provide us with an accurate snapshot of your health, allowing us to create a personalized recommendation for you.

Active Listening

Now it's your turn. Share your health goals and challenges with us. Let us know what you aim to achieve, how you plan to get there, and your desired timeframe. Tell us if you prefer to navigate this journey alone or if you'd like to involve family and friends. It’s also important for us to understand your financial considerations regarding your medical care.

We genuinely want to get to know you and your experiences as we identify any underlying root causes that may be contributing to or worsening your issues.

Monitoring Setup

We'll initiate an at-home monitoring program tailored to your medical needs:

  • Blood glucose using a CGM (continuous glucose monitor) or a BGM (blood glucose monitor)

  • Blood pressure using a BPM (blood pressure monitor)

  • Weight using a scale

These devices will start transmitting your biometrics to us in near real-time, allowing us to monitor your progress, proactively manage your health, and implement meaningful changes as needed.

Your #reach7 Recommendations

We'll close with an individualized plan to begin achieving your objectives. The journey may include specific medication changes, lifestyle modifications, nutritional tweaks, hydration adjustments, sleep improvements, stress management, further medical testing to be conducted, and/or additional medical specialists to be involved.

We'll get on a regular cadence to monitor your progress and move the needle towards your health goals. Whether it's in-person, via telehealth/phone, or through our app for your convenience, we've just embarked on the journey together to help you Live Better & Thrive Longer.

We’ll tackle each one at a pace that feels right for you.

This could be weekly, biweekly, monthly, or even quarterly.
